Putting Abilities to Work
True Heart supports adults with disabilities through our Ready & Able program to assist them in finding integrated, competitive and meaningful employment that align with their career interests, experiences and goals. We work with adults of all abilities who may have an intellectual disability, physical disability and neuro-diverse clients on the autism spectrum. We support our clients in a comprehensive process guided by a person-centered strengths-based approach.

Psychiatric Rehabilitation Program
PRP
community-based and trauma-sensitive services for minors and adults in need of behavioral health supports
The focus of our PRP service is to assist individuals in identifying and addressing mental health barriers by developing and implementing a person-centered rehabilitation program.
For minors and adults that we serve, our PRP services are intended to:
Minors (under the age of 18)

Promote resiliency.

Facilitate the development or restoration of appropriate skills in an individual younger than 18 years of age including but not limited to self-care skills, social, peer, family, and teacher interactions; and semi-independent living skills; and

Promote successful integration into the community and the use of community resources.
Adults (age 18 and over)

Provide community-based comprehensive rehabilitation and recovery services and supports, including, but not limited to community living skills, activities of daily living, and family and peer support

Promote successful community integration and use of community resources
Gifted and Purposed to Shine: Program Model. Our GPS model facilitates our service to minors (ages 5-17) and adults (18 and over) in supporting them in achieving their rehabilitation goals. We believe that each individual we serve has gifts and talents that can either be discovered or nurtured to aid them in living their best lives with emotional and spiritual peace.
