Programs & Services We Provide
In our service to children, youth, adults and communities - True Heart offers programs and services intended to facilitate positive changes for those we support. Our services focus on addressing the emotional, systemic and societal issues that hinder growth - for growth.
Youth Violence Prevention
Don't Die Young is a youth violence prevention promotion initiative designed to serve black and brown boys and youth most impacted by violence.
The model for Don't Die Young is built on a platform to partner with youth, community based organizations and other community stakeholders to promote information and inspiration.
Supported Employment
Every individual has skills (discovered or undiscovered) that should afford them access to meaningful employment. True Heart's Supported Employment program offers a customized approach in helping adults explore employment goals along with a plan to help them achieve these goals via a person centered approach. As part of this service, we assist clients with their resumes, cover letters, job search, job matching and employment retention.
Behavioral Health - PRP Services
True Heart serves adults and minors who have a mental health diagnosis in achieving their transformational rehabilitation goals. Through a person-centered process that focuses on the gifts, talents and interests of those we serve, our Psychiatric Rehabilitation Program (PRP) serves as a community based resource in partnership with licensed mental health professionals and other stakeholders.
